We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will be responsible for providing high-quality nursing care to patients in post-acute facilities. With your expertise, you will monitor patients' physical and mental conditions, track their vital signs, and perform resident intakes, documenting medical histories and health concerns.

You will also help with medication administration and verification, cultivate a safe and comfortable facility for residents, and track medical records and charts. Additionally, you will assist with IVs, immunizations, and phlebotomy procedures, and maintain medical supplies and inventory. Your ability to follow resident care plans and ensure adherence is crucial to the success of this role.

To be successful in this position, you must have a current Massachusetts state license to practice as a Licensed Practical Nurse (LPN), completion of an approved LPN educational program, and a high school diploma or GED/equivalent. A minimum of 6 months of experience (LPN or related role) is required, as well as the ability to lift a minimum of 50 lbs.
