In the home health care setting, a Licensed Practical Nurse (LPN) provides one-on-one skilled nursing care and treatment to patients in their own homes. This role involves assessing patient needs and developing a personalized care plan with the physician, as well as carrying out the care plan with the patient, family, and support staff.

Key Responsibilities
  • Assess patient needs and develop a plan of care with the physician
  • Carry out the care plan with patient, family, and support staff
  • Monitor and evaluate patient response to treatment, medication, and healing or mobilization progress
  • Oversee case management
Qualifications

To be successful in this role, you will need:

  • Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing
  • 2 years Practical Nursing Experience (Preferred)
Certifications and Licenses

This role requires:

  • Licensed Practical Nurse (LPN) with current Georgia license
  • Basic Life Support (BLS)

Preferred certifications include Advanced Cardiac Life Support (ACLS). A valid Georgia Driver's License is also required.
