Per Diem LPN/LVN Home Health Nurse

BAYADA Home Health Care is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a per diem home health nurse. This is a unique opportunity to provide high-quality, patient-centered care to adults in the comfort of their own homes.
Key Responsibilities- Make home visits to clients living in designated territories, providing personalized care and support.
- Perform a range of duties, including administering medication, wound care, treatments, and procedures, as prescribed by physicians.
- Monitor clients' conditions, reporting any changes to the Clinical Manager and Client Services Manager.
- Follow up with, execute, and properly document doctors' orders, ensuring accurate and timely care.
- Perform client assessments as necessary, identifying areas for improvement and developing effective care plans.
- Case management and coordination, working closely with healthcare teams to ensure seamless care transitions.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.
- A current license as a Licensed Practical Nurse in the state of VA.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Prior home care experience strongly preferred, but not required.
- Ability to work independently and manage time effectively, prioritizing tasks and meeting deadlines.
- Strong interpersonal skills, with the ability to build rapport with clients, families, and healthcare teams.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned, with a valid driver's license and reliable transportation.
