Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Open Systems Healthcare. As a Registered Nurse, you will be responsible for providing high-quality patient care and services in the comfort of the consumer's home and community.

Key Responsibilities:

  • Conduct thorough assessments of patient conditions and develop individualized care plans to meet their unique needs.
  • Provide skilled care services, including wound care and tube feeding, under the direction of the Director of Nursing and per the physician's order.
  • Collaborate with other healthcare professionals to identify resources available and ensure access to information for consumers.
  • Document all interactions and care services provided in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
  • Communicate effectively with internal clinicians and external stakeholders to ensure seamless care coordination.

Requirements:

  • Current RN license and BSN degree.
  • 1 year of supervisory experience as an RN.
  • Homecare experience preferred but not required.
  • Evidence of Tuberculosis screening that meets CDC guidelines.
  • Current and valid CPR certification.
  • Reliable form of transportation.
