About the Role: As a Personal Care Specialist at Aveanna Healthcare, you will play a vital part in providing one-on-one care for patients transitioning from hospital to home. Your primary responsibility will be to ensure these individuals receive continuous support in a familiar environment. Collaborating closely with healthcare teams, you will implement tailored care plans, monitor patient conditions, and administer medications as prescribed. Responsibilities: Your key responsibilities will include:
  • • Implementation of physicians' prescribed plans of care
    • Patient assessments and coordination of care
    • Administration of prescribed medication, treatments, and therapies
    • Compliant, timely documentation of care provided in system of record
    • Health promotion, teaching, and training of family members
    • Accepting and writing verbal orders from physicians as required, with co-signature of Case Manager
    • Follow-up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition
    • Performing other appropriate duties as assigned

Qualifications: To be considered for this role, you must possess the following qualifications:
  • • Active, unencumbered license (RN/LPN/LVN) in the state where you'll practice
    • Current CPR certification from American Red Cross, American Heart Association, or BLS
    • TB skin test (current within last 12 months)
    • Six months prior hands-on nursing experience preferred but not required
    • Reliable transportation

Salary Range: $60,000 - $80,000 per year, depending on experience.
