Job Title: Licensed Practical Nurse

Avamere Health Services is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Laurelhurst Village in Portland, OR.

About the Role:

We are looking for a dedicated and caring LPN to provide high-quality patient care in our skilled nursing facility. As an LPN with Avamere, you will have the opportunity to work with a diverse range of patients and residents, providing individualized care and support to meet their unique needs.

Responsibilities:
  • Administer medications and review medication cards for completeness and accuracy.
  • Develop and implement individualized care plans for residents, in collaboration with the interdisciplinary team.
  • Provide direct care to residents, including bathing, dressing, and grooming.
  • Conduct regular assessments and evaluations of residents' physical and emotional needs.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
Requirements:
  • Current, unencumbered LPN license in the state of Oregon.
  • Experience working in a skilled nursing facility or similar setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
