Senior Hospice Nurse Care Coordinator

We are seeking an experienced Senior Hospice Nurse Care Coordinator to join our team at Cantex. This is a challenging and rewarding role that requires strong clinical skills, excellent communication abilities, and a passion for providing high-quality patient care.
About the RoleThe successful candidate will be responsible for coordinating care for patients with terminal illnesses, working closely with interdisciplinary teams to develop and implement comprehensive care plans. They will also provide education and support to patients and their families, promoting independence and dignity throughout the care process.
Key Responsibilities- Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs
- Develop and implement individualized care plans in collaboration with healthcare professionals and family members
- Provide education and support to patients and their families on disease management, end-of-life care, and self-care techniques
- Collaborate with other healthcare providers to ensure seamless care coordination and timely interventions
- Document all interactions, assessments, and care planning in accordance with regulatory requirements
- Bachelor's degree in Nursing (BSN) or Master's degree in Nursing (MSN)
- Minimum 2 years of experience as a Registered Nurse (RN) in a hospice or palliative care setting
- Strong clinical knowledge and skills in gerontology, oncology, and palliative care
- Excellent communication and interpersonal skills
- Able to work independently and collaboratively within a team environment
Cantex offers a highly competitive compensation package, including:
- $85,000 - $110,000 per year (dependent on experience)
