Job Summary:

As a skilled nursing professional at HexaQuEST Health, Inc., you will be responsible for ensuring the continuity of care for our residents. This includes providing direct care and supervising the care given by CNAs, LVNs, RNs, and support staff members. You will communicate effectively with physicians and the healthcare team about changes in patient condition, including results of diagnostic studies and symptomology. Additionally, you will participate in Quality Improvement activities and assess, plan, implement, and evaluate the nursing care of residents within the skilled nursing setting.

Responsibilities:

Assess and plan the nursing care of residents
Provide direct care and supervise the care given by CNAs, LVNs, RNs, and support staff members
Communicate effectively with physicians and the healthcare team
Participate in Quality Improvement activities
Maintain accurate and up-to-date records of patient care

Requirements:

Current California RN license
Minimum two (2) years prior experience in a long-term care facility
Working knowledge of Title 22, OBRA, and other regulations
Operating knowledge of Electronic Medical Records preferred
Must pass nursing medical test and have current BLS certification
Must be able to effectively communicate in English, both verbally and in writing
Must have good interpersonal skills and interface professionally, tactfully, and respectfully with residents, patients, staff, the general public, and governmental agency representatives
Basic computer skills required

Working Conditions:

This position requires working in a fast-paced environment with frequent interruptions. The ability to multitask and prioritize tasks effectively is essential. The successful candidate will be able to work independently and as part of a team, with a strong focus on providing high-quality patient care.
