Job Title: Registered Nurse (RN)

This is an exciting opportunity for a skilled and compassionate Registered Nurse (RN) to join our team as a key member providing clinical services to clients in their homes.

Key Responsibilities:
  • Develop individualized care plans based on comprehensive assessments of clients' health needs and goals.
  • Administer medications, treatments, and other nursing procedures as required, ensuring the highest level of patient safety and satisfaction.
  • Monitor clients' vital signs, progress, and overall health status, adjusting care plans as necessary to ensure optimal outcomes.
  • Educate clients and their families on disease management, medication administration, and home safety practices, promoting independence and self-care.
  • Perform wound care, IV therapy, catheter care, and other advanced nursing procedures as needed, staying up-to-date with best practices in nursing and home care.
  • Maintain accurate and timely documentation of all care provided, adhering to company and regulatory guidelines.
  • Ensure compliance with state and federal healthcare regulations and standards, upholding the highest standards of professionalism and integrity.
  • Provide emotional support and guidance to clients and their families, helping them navigate the challenges of home healthcare and promoting overall well-being.
Requirements:
  • Active and unrestricted RN license in the state of New York.
  • Minimum of 1-2 years of nursing experience, preferably in home health, geriatrics, or a related field.
  • Strong clinical skills with the ability to perform a wide range of nursing procedures independently, requiring minimal supervision.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and the healthcare team.
  • Ability to work independently in a home care setting, managing time and resources effectively to meet client needs and expectations.
  • Reliable transportation and a valid driver's license, allowing for easy travel to client locations.
  • Current CPR and BLS certification, reflecting a commitment to patient safety and well-being.
  • Familiarity with electronic health records (EHR) systems, enhancing efficiency and accuracy in documentation and care planning.
