Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Professional. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary:

The Registered Nurse will be responsible for providing high-quality, patient-centered care to individuals in their homes. This includes assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans that address patients' physical, emotional, and social needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current RN licensure in the State of New Jersey.
  • At least 1 year of experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
