- Facilitates the formulation and execution of patient evaluations in line with federal and state regulations as well as organizational objectives and standards.
- Conducts and/or organizes patient and family assessment discussions, creating care plans that align with clinical needs, risk factors, and preferences.
- Arranges and leads meetings with family members and interdisciplinary team members to review patient care strategies, specific requirements, and suitable interventions.
- Performs regular physical assessments of patients and oversees the accuracy of the Minimum Data Set (MDS) process.
To qualify for this position, candidates must:
- Hold a valid Registered Nurse license from the Georgia State Nursing Board and possess proven expertise in data entry, MDS coding, and the overall RAI process.
- Uphold the standards, values, and principles of the organization.
- Exhibit strong verbal and written communication skills.
- Demonstrate the ability to foster effective working relationships with patients, family members, department leaders, and colleagues.
