Senior Care Coordinator

Rome Health is seeking a compassionate and dedicated Registered Nurse to fill our MDS Coordinator position. This individual will play a vital role in providing top-notch care to our residents at the Residential Health Care Facility.
About the Role:
As a Senior Care Coordinator, you will be responsible for ensuring accurate completion of the Minimum Data Set (MDS) per state and federal guidelines. You will also educate and train new staff on MDS completion, complete MDS accurately per RAI manual, and work alongside billing to ensure compliance.
Requirements:
To excel in this role, you should possess MDS Certification, a current NY State RN license, and a BSN or bachelor's degree. Experience in the long-term care setting, strong interpersonal and communication skills, and a strong understanding of nursing home regulations and reporting are also essential.
