Senior Clinical Care Coordinator

About the Role

Our client, a leading healthcare provider in Clinton, Maryland, is seeking a skilled Senior Clinical Care Coordinator to join their team. As a key member of the care coordination team, you will be responsible for ensuring seamless transitions of care for patients across various settings. Your primary focus will be on developing and implementing individualized care plans that cater to the unique needs of each patient.

Responsibilities

- Collaborate with healthcare providers, patients, and families to identify and address care gaps and plan for post-acute care.
- Develop and implement comprehensive care plans that meet the needs of patients with complex medical conditions.
- Coordinate with internal and external stakeholders to ensure effective communication and timely exchange of information.
- Monitor patients’ progress, identify areas for improvement, and make data-driven decisions to optimize care outcomes.
- Foster strong relationships with patients, families, and healthcare providers to ensure a high level of satisfaction and trust.
- Stay up-to-date with the latest clinical guidelines, best practices, and regulatory requirements to ensure compliance and quality care.

Requirements

- Current RN license in good standing in the state of Maryland or a Compact state.
- Minimum of 1 year of experience in case management or a related field.
- Strong clinical knowledge and expertise in developing and implementing care plans.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced environment with multiple priorities and deadlines.
- Strong analytical and problem-solving skills to identify and address care gaps.
