Clinical Care Specialist

Job Summary:
At Liberty Health, we are committed to delivering exceptional care to our patients and seeking an experienced Clinical Care Specialist to join our team. As a Clinical Care Specialist - Chronic Condition Management, you will play a pivotal role in providing comprehensive care to patients with chronic conditions, ensuring their stability and continuity of care.
Key Responsibilities:
- Conduct thorough assessments of patients with chronic conditions to identify potential complications and develop effective care plans.
- Collaborate with healthcare teams to ensure seamless transitions of care, promoting patient safety and satisfaction.
- Provide education and support to patients and their families on self-management techniques, medication adherence, and lifestyle modifications to improve health outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate exchange of information.
- Document patient care and progress in the electronic medical record, maintaining accurate and up-to-date records.
- Stay current with best practices and evidence-based guidelines for chronic condition management, applying this knowledge to improve patient care.
- Work collaboratively with the interdisciplinary team to develop and implement care plans that address the unique needs of each patient.
- Participate in quality improvement initiatives to enhance patient care and satisfaction, identifying areas for improvement and implementing changes as needed.
Requirements:
- Demonstrated experience in chronic condition management, preferably in a home health or hospice setting.
- Current, unrestricted Registered Nurse license in the state of assignment or the ability to obtain and submit proof of license renewal every other year.
- Certification as a Registered Nurse by a national credentialing body.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities simultaneously.
- Excellent critical thinking and problem-solving skills, with the ability to analyze complex patient data and develop effective care plans.
- Proficiency in electronic medical records and other healthcare software applications.
Salary: $65,000 - $85,000 per year, depending on experience.
