We are seeking a skilled Wound Care Nurse to join our team at Santa Fe Care Center.
Key Responsibilities:
- Complete MDS documentation for Skin Condition (Section M) for all facility residents
- Evaluate the effectiveness of nursing and other clinical education programs
- Attend staff meetings, in-service classes, and committee meetings
- Participate in weekly wound team rounds
- Establish competencies for clinicians regarding wound care
- Collaborate with other members of the Education Department on joint educational initiatives
- Collaborate amongst Interdisciplinary Team to encompass all aspects of care to promote wound healing
- Assist in the implementation of individualized treatment plans for each assigned resident
- Participate in the development and achievement of Wound Care Program and objectives
- Perform all other duties as requested
- Communicate in a positive, caring, and helpful manner with residents, family members, and other staff
- Develops educational modules for wound care to be used for internal and external education, including teaching tools for patients and families
- Respect the dignity of each person
Requirements:
- Ability to lift 50 pounds occasionally, 20 pounds frequently and 10 pounds constantly
- Ability to work and stand 80% of an 8-to-12-hour shift
- Certified BLS or completion in first 90 days of employment required
- Excellent presentation skills and organizational skills required
- WCC certification, CWCN, CWOCN or CWS is preferred or certification pending within one year
- Excellent time management skills
- Ability to perform effectively in fast-paced, stressful environment
- Excellent physical assessment and critical thinking skills
- Excellent communication skills
- Confers with the Director of Nursing and/or other licensed nursing personnel regarding dermatologic disorders of residents in the facility
- Valid State RN License required. Previous geriatric nursing experience preferred
