Job Overview

We are seeking a compassionate and dedicated Bereavement Specialist to join our team at ThedaCare. This role is essential in providing support and guidance to families navigating the difficult journey of loss.

PRIMARY RESPONSIBILITIES:

  • Deliver bereavement education and counseling services to hospice patients and their families who express a need for support based on thorough assessments.
  • Organize, oversee, and facilitate grief support programs and ongoing support groups for those in need.
  • Implement bereavement programming tailored to the hospice program and community educational requirements.
  • Coordinate and manage memorial services for families of patients, held biannually.
  • Ensure accurate documentation in compliance with regulatory and licensure standards for bereavement support lasting one year following the patient's passing.
  • Design and develop educational materials for grief support groups and workshops.
  • Provide care and services that align with the standards set forth by the National Hospice and Palliative Care Organization for bereavement programs.
  • Create assessment tools to identify bereaved individuals in accordance with regulatory guidelines.
  • Recruit, train, and mentor volunteers who assist in bereavement services.

REQUIRED QUALIFICATIONS:

  • A Bachelor's degree in social work, marriage and family therapy, or mental health counseling.
  • A minimum of one year of experience in healthcare, with specialized training in bereavement and loss.
  • Certification or licensure by the State of Wisconsin at the highest level permitted by the individual's qualifications.

PHYSICAL REQUIREMENTS:

  • Ability to perform physical tasks including standing, walking, bending, and lifting up to fifty (50) pounds without assistance.
  • Exposure to bloodborne pathogens while executing job responsibilities.

WORKING CONDITIONS:

  • Must be available to work scheduled hours, with flexibility for shifts that may vary based on patient and business needs, including overtime, on-call, nights, weekends, and holidays.
  • Engage in collaboration with team members and other healthcare professionals.

Weekly Hours: 40

Full-Time Equivalent: 1

Overtime Status: Non-exempt
