About the Role

CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team in providing high-quality care to patients in their homes. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients, helping them to regain their independence and improve their quality of life.

Key Responsibilities
  • Evaluate patients' speech and language skills, identifying areas of strength and weakness, and developing personalized treatment plans to address these needs.
  • Direct and provide speech/language pathology services to patients, using a variety of techniques and strategies to help them achieve their goals.
  • Collaborate with other healthcare professionals, including nurses, physical therapists, and occupational therapists, to ensure comprehensive care and optimal patient outcomes.
  • Participate in the development and periodic review of patients' Plans of Treatment and Plans of Care, ensuring that their needs are met and their progress is tracked.
  • Administer and interpret diagnostic tests, such as audio logic screening, to assess patients' speech and language skills.
  • Provide instruction and training to patients and their families on the use of alternative communication systems, as needed.
  • Document patients' progress, including their responses to treatment, and submit timely written reports to supervisors and physicians.
  • Participate in care coordination activities and discharge planning, ensuring a smooth transition for patients from hospital to home care.
Requirements
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
  • Minimum of six months experience as a speech therapist/speech language pathologist.
  • Home health experience a plus.
  • Current and unrestricted license.
