We are seeking a skilled Pharmacist to join our team at HealthCare Support.

Job Summary:

The successful candidate will be responsible for evaluating physician medication orders, making recommendations for therapy changes, and maintaining patient/resident databases.

Responsibilities:

  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
  • Make recommendations for therapy changes to the physician and facility staff as appropriate.
  • Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support.

Requirements:

  • Bachelor's Degree/Pharm D in Pharmacy.
  • Clear and Active Pharmacist license in the state of Pennsylvania.
  • 1+ year of experience as a Pharmacist.
