We are seeking a highly skilled and experienced Clinical Pharmacist Specialist to join our Oncology team at St. Luke's Hospital. As a key member of our interdisciplinary team, you will provide expert pharmaceutical care to patients receiving antineoplastic therapy or palliative care therapy in the setting of malignancy.
Key Responsibilities- Provide collaborative pharmaceutical care to patients, including medication therapy management and monitoring for adverse drug reactions.
- Work closely with pharmacy leadership, hematology/oncology providers, and infusion centers to monitor chemotherapeutic utilization, supportive drug therapy utilization, and patient and provider safety.
- Develop and implement clinical pathways for oncology services, ensuring evidence-based practice and optimal patient outcomes.
- Collaborate with clinical coordinators to develop and implement clinical activities for pharmacy staff, including education and training programs.
- Provide drug information and participate in medical rounds as needed.
- Assist in developing pharmacy policies and procedures related to anti-neoplastic medications, treatment of malignancy, and safe handling of hazardous medication.
- Mentor pharmacists, pharmacy residents, and students in oncology clinical practice.
- PharmD degree required.
- ASHP accredited PGY-1 Residency required.
- ASHP accredited PGY-2 Residency in Oncology or equivalent experience required.
- Registered in Pennsylvania or completion of Boards with pending licensure may be accepted.
- State mandated continuing education required.
This position requires sitting for up to 8 hours per day, standing for up to 8 hours per day, and walking for up to 8 hours per day. The employee must be able to continuously finger, frequently handle, firm grasp, twist, and turn. The employee must also be able to frequently reach above shoulder level, look up, and occasionally lift up to 20 pounds and carry up to 35 pounds and push up to 200 pounds. The employee must be able to occasionally stoop, bend, squat, crouch, and kneel. The employee must be able to rarely crawl and climb. The employee must be able to hear as it relates to normal conversation, low and high frequencies, and see as it relates to general vision, near vision, far vision, depth perception, peripheral vision, color vision, and monotony.
