In this challenging and rewarding role, you will have the opportunity to make a meaningful difference in the lives of HIV positive patients. As a Nurse Care Manager at The Institute for Family Health, you will work collaboratively with an interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient. Your responsibilities will include meeting with patients to assess their medical needs, performing clinical nursing duties, such as triaging patients and administering medications, and collaborating with medical providers and other healthcare professionals to coordinate patient care. You will also play a critical role in screening patients for mental health and substance use treatment needs and referring them to necessary services. In addition to these responsibilities, you will provide individual and group-based health promotion, treatment adherence counseling, risk reduction counseling, and case management to assist patients in decreasing barriers to viral load suppression and retention in care. Your expertise will be essential in reviewing health maintenance topics and outreach patients as needed for compliance. To excel in this role, you must possess a valid NYS nursing license and computer literacy with proficiency in MS Word, Excel, and PowerPoint. You should also demonstrate strong organizational, interpersonal, oral, and written communication skills, the ability to handle multiple assignments, generate, interpret, and analyze data from multiple sources, and bi-lingual skills, preferably Spanish. With an estimated salary range of $43.00 - $45.56 hourly, this role offers a competitive compensation package.
