Job Summary

The Physical Therapist will provide physical therapy services to patients and adhere to all conditions outlined in the Personnel Services Agreement. This role is responsible for assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. The Physical Therapist will also direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.

Essential Job Functions

  • Assess and evaluate patients' therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
  • Direct physical therapy treatment and instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy and report to the physician patient's reaction to treatment or changes in condition.
  • Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
  • Train patients in the use of prosthetic devices and identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals and participate in discharge planning for patients.
  • Provide physical therapy consultation to home families/caregivers when indicated and provide inservice education programs for nursing organization personnel as needed.
  • Participate in peer consultation processes and supervise physical therapy assistants according to organization policy and state regulations.


Position Qualifications

  • Possess a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the state, if applicable.
  • Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
  • Demonstrate good verbal and written communication and organization skills.
  • Possess and maintain current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
