Job Overview

Position Summary

The Licensed Practical Nurse (LPN) (Certified) plays a vital role in delivering compassionate nursing care to elderly residents who are acutely ill or injured. Under the supervision of a Registered Nurse (RN), the LPN integrates both technical skills and theoretical knowledge to cater to the unique needs of residents and their families. This position emphasizes the importance of preventing illness and injury, maintaining wellness, and ensuring comfort for all residents.

Key Responsibilities

  • Deliver direct care and assist with nursing tasks as directed, adhering to established scope of practice and competency standards. Collaborate with the RN for assessments and recommendations, and participate in resident evaluations and care planning.
  • Administer treatments and medications as prescribed, ensuring compliance with physician orders and maintaining accurate documentation of care provided.
  • Monitor and document resident status, ensuring that all clinical records are complete and accurate in accordance with company and regulatory standards.
  • Engage in medication management to enhance safety and effectiveness, including administering medications, observing resident responses, and maintaining accurate narcotic records.
  • Assist with the clinical admission process for new residents and follow up on consultant recommendations.
  • Participate in the clinical discharge process and perform other nursing duties as required.
  • Ensure punctuality and regular attendance for assigned shifts.

Qualifications

  • Education: Graduate of an accredited LPN School of Nursing.
  • Licenses/Certification: Current valid LPN license in the state of employment, valid CPR certification, and current certification in a clinical specialty area such as long-term care, gerontology, wound care, IV therapy, ventilator care, or hospice and palliative care.
  • Experience: Preferred six months of experience in a long-term care setting.
