Case Manager Continuing Care Specialist

As a Case Manager Continuing Care SW, you will play a critical role in coordinating patient care and population-based management for patients in defined geriatric or other patient populations. You will work with physicians, staff, and non-Kaiser providers to plan and implement a comprehensive, multi-disciplinary approach to manage health conditions, utilization of resources, and patient self-care.
Key Responsibilities- Plan, develop, assess, and evaluate care provided to members.
- Develop and implement individualized patient care and treatment plans in conjunction with primary care and specialist physicians.
- Recommend alternative levels of care and ensure compliance with federal, state, and local requirements.
- Assess physiological and functional status using established protocols and initiate diagnostic testing and interventions as needed.
- Develop and deliver patient and family education plans focused on self-management and disease-specific education.
- Implement strategies to target and assess risk factors and achieve patient follow-up according to clinical and strategic measures.
- Produce population-based reports on outcomes specific to defined patient populations.
- Participate in the healthcare team to plan, evaluate, and implement decisions and strategies to achieve predetermined cost, clinical, quality, utilization, and service outcomes.
- Develop and maintain case management policies and procedures.
- Coordinate care and services with utilization and quality reviewers and monitor the level and quality of care.
- Graduate of an academic institution accredited by the Council on Social Work Education and a Master's degree in Social Work.
- Demonstrated knowledge of case management, discharge planning, transfer coordination, and TJC and other federal/state/local regulations.
- Ability to work in a Labor/Management Partnership environment.
