Hospice Case Manager

Job Summary:
The Hospice Case Manager Registered Nurse is responsible for planning, organizing, and directing hospice care services to meet the needs of individuals and families within their homes and communities. This position requires a strong background in nursing, with emphasis on community health education and experience.
Key Responsibilities:
- Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
- Provide professional nursing care by utilizing all elements of the nursing process.
- Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed by the physician.
- Counsel patients and families in meeting nursing and related needs, provide health care instructions, and assist with activities of daily living.
- Act as Case Manager when assigned, coordinate patient care for assigned caseload, and complete, maintain, and submit accurate and relevant clinical notes.
- Communicate with physicians, community health-related persons, and other team members to coordinate care plans and provide continuity of care.
Requirements:
- At least 18 years of age.
- Required Health Screening, TB Clearance, and Criminal Record Clearance.
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for Nursing accredited school of nursing.
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Preferred Qualifications:
- A minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing.
