Key Responsibilities:

- Conduct thorough assessments through structured interviews with caregivers and observations in various settings, including home and community.
- Analyze social history and complete functional assessments to establish behavior baselines for individuals.
- Collaborate with interdisciplinary teams to present assessment summaries and recommend behavior support plans.
- Advocate for the rights of individuals receiving services, participating in review committees as necessary.
- Maintain accurate documentation of progress, service changes, and billing, while ensuring confidentiality and compliance with all regulations.
- Foster positive relationships with individuals, families, and case managers, adhering to customer service standards.
- Report any incidents of abuse or neglect in accordance with established protocols.
- Support staff in implementing Individual Support Plans and conduct training sessions as required.
- Assist individuals with daily living activities, including personal care, socialization, and community engagement.
- Ensure safety during transportation and assist with medical appointments as needed.

Qualifications:
- Bachelor's Degree in a relevant human services field.
- Minimum of two years of related experience, with training in behavior modification techniques.
- Knowledge of specialized populations, including those with developmental disabilities or mental health challenges.
- A reliable and compassionate approach, with a commitment to the organization's mission and values.
- Valid driver's license and completion of all state-required training and certifications.
