About Summit ElderCare
Fallon Community Health Plan operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Our PACE program, Summit ElderCare, provides a welcome alternative to nursing home care for older adults and their caregivers.
Brief Summary of Purpose
As a Home Care Nurse - PACE Program, you will be responsible for the effective management and delivery of direct nursing care to PACE participants in any setting, utilizing the nursing process and adhering to standards of nursing practice.
Key Responsibilities
- Review and implement provider orders in a timely and accurate manner.
- Conduct nursing assessments according to policy guidelines, including physical, psychosocial, behavioral, and MDS-HC as indicated.
- Involve participants and significant other(s) based on needs and abilities.
- Deliver care to participants in the home setting, including skilled services, based on individualized needs and according to age-appropriate nursing standards.
- Provide culturally sensitive care to participants.
- Monitor and evaluate therapeutic interventions.
- Participate in the development and ongoing review of each participant's care plan.
- Delegate and accept care responsibilities to other members of the healthcare team, when appropriate.
- Identify emergency situations and initiate appropriate nursing orders/interventions.
- Meet the needs of participants in a timely manner.
- Participate in the interdisciplinary team (IDT) process and collaborate with IDT members to meet the needs of participants.
- Consistently document all aspects of participant care, including significant changes in health status, monthly nurses' notes, and healthcare teaching in the medical record.
- Participate in training and orientation of new nursing staff as assigned.
- Assist in the delivery of other nursing services as assigned.
- Participate in the nursing on-call rotation as assigned.
- Initiate and complete falls, infection, grievance, and incident reports.
- Active participant in site-specific committees.
- Participate in the post-IDT huddle and any other additional meetings when required.
- Perform all duties in accordance with Fallon Community Health Plan and Summit ElderCare policies and procedures.
- Function independently to administer medications and treatments as ordered by the provider.
- Function independently in the home care nurse role.
- Perform a nursing assessment and execute the plan of care in all settings the participant may be in.
Qualifications
- Graduate of an accredited school of nursing.
- Current license as a RN in the state of Massachusetts.
- CPR and Alzheimer's certification, or willingness to be certified within 60 days of hire, is essential.
Experience
- At least two years of recent experience in the direct care of adults or chronically disabled persons with at least one year caring for a frail or elderly population.
- Ambulatory care or home care experience is helpful.
Other
- Reliable transportation is required.
