We are seeking a highly skilled Clinical Case Manager to join our team at Christus Health. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.
Key Responsibilities- Conduct thorough assessments of patients' medical and social needs to develop effective care plans.
- Collaborate with healthcare providers, patients, and families to ensure timely and efficient discharge planning.
- Utilize nationally recognized medical necessity criteria to conduct utilization reviews and document findings.
- Refers cases for second-level reviews when medical necessity criteria are not met.
- Stay up-to-date with regulatory requirements, including Medicare Conditions of Participation, DNV standards, HIPAA, and reporting requirements.
- Communicate effectively with patients, families, and healthcare providers to ensure clear understanding of care plans and progress.
- Participate in interdisciplinary treatment planning rounds to ensure comprehensive care.
- Develop and implement innovative methods to facilitate discharge planning and patient education.
- Associate of Science degree required; Bachelor of Science degree preferred.
- Licensed Registered Nurse (L-RN) with Single-state licensure required; Multistate licensure and C-BLS certification preferred.
