The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Assess clients' physical, emotional, and social needs
  • Develop and implement individualized care plans
  • Provide direct care services, including wound care, medication management, and education
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Document client progress and care plans

Requirements:

  • Current RN licensure in the State of Nevada or California
  • At least one year of acute care experience and/or one year of home health experience
  • Ability to obtain and maintain a valid driver's license
  • Current BLS certification
  • Proficiency in Microsoft Office Suite

Working Conditions:

This position requires the ability to work independently and as part of a team, with frequent travel to clients' homes. The incumbent must be able to lift up to 25 pounds and stand for long periods.
