Hospice Nurse Practitioner

We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team at University of TN Medical Center Hospice Services, a part of LHC Group. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on symptom management, supportive care, and palliative care.
Responsibilities- Make face-to-face visits to qualifying Hospice patients, reporting clinical findings to the Hospice Medical Director and collaborating with the patient's primary physician.
- Consult with the Hospice Medical Director as needed, informing primary physicians of recommended services and collaborating with other physicians as necessary.
- Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care.
- Instruct and supervise patients and families in self-care techniques when appropriate.
- Maintain accurate and relevant clinical notes regarding patient condition and report patient condition and identified needs/issues to Administrator and Hospice Medical Director.
- Minimum of five years nursing experience, with clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred.
- Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Current certification by a national body such as ANCC, NBCHPN, or AANP.
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
- Current CPR certification required.
