Job Summary

We're seeking a skilled RN Care Manager to join our team. As a key member of our interdisciplinary team, you'll assess, plan, advocate, and coordinate care for patients from admission to discharge. Your goal will be to ensure a safe transition post-hospitalization and optimize health outcomes.

Responsibilities
  • Assess patients' health needs and develop individualized care plans
  • Coordinate services to optimize health outcomes
  • Communicate effectively with patients, families, and healthcare providers
  • Advocate for patients' needs and promote health education
  • Facilitate smooth transitions of care across healthcare settings
Requirements
  • Graduate of an accredited School of Nursing
  • 1 year of acute care hospital setting experience
  • Current RN License in the state of employment
  • BLS (Basic Life Support) certification
Preferred Qualifications
  • Certification in Case Management
  • 2 years of acute care hospital setting experience
