Key Responsibilities:

  • Assessment and Planning: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing personalized care plans to meet their unique requirements.
  • Medication Management: Administer medications as prescribed by physicians, monitor patient responses, and report any adverse reactions or concerns.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date medical records, including intake and output records, nurses' notes, and physicians' orders.
  • Collaboration and Communication: Work closely with physicians, directors, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Emergency Procedures: Respond promptly and professionally to emergency situations, including cardiopulmonary resuscitation and notification of family and third parties in the event of a patient's passing.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and satisfaction, and contribute to the development of policies and procedures to ensure compliance with regulatory requirements.

Requirements:

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Minimum of 1 year of experience in a healthcare setting, preferably in a similar role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
