Job SummaryThe Licensed Practical Nurse (LPN) plays a vital role in delivering compassionate care to our residents. As a member of our interdisciplinary care team, you will be responsible for assessing residents' needs, administering medications and treatments, and documenting interactions and outcomes.
To excel in this role, you should possess excellent communication skills, empathy, and a strong ability to work effectively with healthcare professionals and the public. Key Responsibilities:- Assess residents' physical and psychosocial needs, developing personalized care plans to meet their unique requirements
- Administer medications and treatments as ordered by healthcare professionals
- Document resident interactions, including outcomes related to medication, behavior, activity, and dietary needs
- Provide technical support to healthcare professionals as needed
- Maintain accurate and up-to-date records of resident care and interactions
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing is preferred
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possesses multi-state licensure privileges
- Obtains and subsequently maintains required department-specific competencies and certifications
