Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in delivering compassionate care to our residents. As a member of our interdisciplinary care team, you will be responsible for assessing residents' needs, administering medications and treatments, and documenting interactions and outcomes.

Key Responsibilities:
  • Assess residents' physical and psychosocial needs, developing personalized care plans to meet their unique requirements
  • Administer medications and treatments as ordered by healthcare professionals
  • Document resident interactions, including outcomes related to medication, behavior, activity, and dietary needs
  • Provide technical support to healthcare professionals as needed
  • Maintain accurate and up-to-date records of resident care and interactions
Requirements:
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse
  • Associate degree in practical nursing is preferred
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possesses multi-state licensure privileges
  • Obtains and subsequently maintains required department-specific competencies and certifications
