About the Role:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our care team at GSS SD Scotland Ctr in Scotland, SD. As an LPN, you will play a vital role in providing high-quality nursing care to our residents, promoting their health, comfort, and well-being.

Job Summary:

The successful candidate will provide professional nursing care for residents of all ages in long-term care settings, working under the supervision of a registered nurse, advanced practice provider, or physician. You will be responsible for assessing residents' physical and psychosocial needs, developing and implementing individualized care plans, and delivering evidence-based interventions to improve resident outcomes.

You will work collaboratively with the interdisciplinary care team to ensure seamless transitions of care, coordinate services, and promote a culture of safety and quality. Your excellent communication and interpersonal skills will enable you to build strong relationships with residents, families, and healthcare professionals, fostering a supportive and inclusive environment.

  • Provide direct patient care, including administering medications, performing treatments, and documenting resident interactions">
  • Contribute to the assessment and development of individualized care plans, incorporating resident preferences and goals">
  • Collaborate with the care team to identify and address resident concerns, ensuring timely intervention and resolution">
  • Maintain accurate and comprehensive documentation of resident information, adhering to regulatory requirements and departmental standards">
  • Participate in quality improvement initiatives, contributing to the development and implementation of strategies to enhance resident care and satisfaction">

Requirements:

To be considered for this rewarding opportunity, you must possess a current unencumbered Licensed Practical Nurse (LPN) license and meet the educational and experience requirements outlined below:

Education: Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred.

">

Experience: A minimum of 400 hours of experience as an LPN at another facility or completion of a pre-licensure nursing program that included supervised clinical experience across the life span.

">

Certifications: Orients to required department-specific competencies and certifications, completes annual continuing education units (CEU), and maintains licensure with the State Nursing Board.

">

Compensation and Benefits:

We offer an attractive benefits package, including competitive salary, comprehensive health insurance, paid time off, and opportunities for professional growth and development. This position may qualify for a sign-on bonus of $10,000.

Estimated Salary: $42,500-$52,000 per year, depending on experience and qualifications.
