Job Title: Case Manager/Client Care Coordinator

Volunteers of America-Greater New York is seeking a skilled Case Manager/Client Care Coordinator to join our team. As a key provider of services, you will work with clients to assess their strengths and needs, plan a course of action, link them to services, counsel them, and monitor progress.

Responsibilities:
  • Manage client intake and assessment process to orient clients to the program and produce a comprehensive assessment of client service needs.
  • Develop individual service plans with clients, reflecting assessment findings and goals, and update plans as required.
  • Meet regularly with clients to assist them in obtaining needed services and benefits to achieve their individual service plan goals.
  • Provide supportive counseling to strengthen clients' ability to make appropriate life decisions.
  • Document case management activities in accordance with agency and funder guidelines and procedures.
  • Collect service data and participate in evaluation and quality assurance activities.
  • Provide aftercare to assigned clients who have been placed or discharged from the facility.
Requirements:
  • Bachelor's degree or 60 college credits.
  • Two years of relevant experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills.
