Key Responsibilities

  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare providers, patients, and families to ensure seamless care coordination.
  • Conduct thorough assessments to identify patient needs and develop effective interventions.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure timely and effective care.

Requirements

  • Graduate from an accredited school of nursing (RN) required, bachelor's degree preferred.
  • Minimum two years of clinical experience, preferably in acute care or community social agency case management.
  • Case Management Certification (CCM) preferred.
  • Washington State Registered Nurse (RN) license and Washington State Driver's License with liability insurance required.
  • Bilingual and/or English-Spanish speaking abilities preferred.
