Job Title: Private Duty Nurse LVN

We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team at Quality Talent Group. As a Private Duty Nurse LVN, you will provide high-quality patient care in the comfort of their own homes, working closely with our patients and their families to ensure the best possible outcomes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing them.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, including vital signs, medication administration, and any changes in condition.
Requirements:
  • Current, unencumbered LPN/LVN license in the state of practice.
  • Minimum 6 months of experience as an LPN/LVN, preferably in home health care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to lift 50 pounds and stand for long periods, with frequent bending, stooping, and reaching.
  • Valid driver's license and reliable transportation.
