RN Patient Navigator

As a RN Patient Navigator - Oncology at LifeBridge Health, you will play a vital role in providing professional nursing care for oncology patients. Under general supervision, you will demonstrate knowledge of age-specific differences in the patient population and ensure that all services are delivered in accordance with the mission statement and SPIRIT values of Carroll Hospital Center.
Responsibilities- Provide professional nursing care for oncology patients following established standards and practices.
- Ensure that all services are delivered in accordance with the mission statement and SPIRIT values of Carroll Hospital Center.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Provide education and support to patients and their families.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
- 4-7 years of Oncology Nursing experience required; Outpatient setting experience preferred.
- CPR - AHA Healthcare Provider certification required.
- ONS - Chemo, Biotherapy Certification required.
- RN - Registered Nurse certification required.
- OCN - Oncology Certified Nurse certification preferred.
