We are seeking a skilled Nurse Navigator to join our team at ChristianaCare. As a Nurse Navigator, you will play a critical role in coordinating care for patients and their families, ensuring seamless transitions and optimal outcomes.
Key Responsibilities:- Coordinate referrals and facilitate communication among healthcare providers to prevent fragmented or delayed care.
- Assess patient and family learning needs to identify barriers to education and develop personalized plans to address these needs.
- Collaborate with multidisciplinary teams to develop and implement patient care plans, ensuring that patients receive comprehensive and coordinated care.
- Provide education and support to patients and families, empowering them to self-advocate and make informed decisions about their care.
- Monitor progress and outcomes, identifying opportunities for improvement and contributing to the development of new processes and protocols.
- BSN required; MSN preferred.
- RN license or eligible for licensure in the state of Delaware.
- Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferred.
- Nursing Cert - Certified Hospice and Palliative Nurse (CHPN) - Hospice & Palliative Credentialing Center within 3 years of hire.
