About the Role

This position will be split between the Greenville OBGYN office and the Lakeview Family Medicine office. As a Social Worker in our Ambulatory Care Management Department, you will play a vital role in identifying and connecting with patients who have behavioral health concerns or chronic medical conditions on an outpatient basis.

About the Department

The Corewell Health West Ambulatory Care Management Department is a dynamic team of Social Workers, RN Care Managers, Community Health Workers, and Medical Assistants. We have a large team of Social Workers and RN Care Managers embedded in Primary Care offices across the system, working together to provide comprehensive care to our patients.

Scope of Work

As a Social Worker in this role, you will:

  • Assess internal referrals and provide individual, short-term therapy services or care management services, monitoring patient progress.
  • Develop plans of care and make recommendations to PCPs, specialists, and other members of the healthcare team regarding mental health treatment strategies.
  • Assess and provide follow-up for the Systematic Case Review process, consulting with psychiatrists and serving as a liaison between PCPs and consulting psychiatrists to assist in psychotropic medication management.
  • Respond to patients' urgent mental health concerns, including suicide risk assessments, and assist patients in accessing appropriate levels of care.

Qualifications

To be successful in this role, you will need:

  • A Master's Degree in Social Work (MSW) - Required
  • 2 years of relevant experience in a related field - Required
  • Licensed Master Social Worker (LMSW) in the State of Michigan - Upon Hire Required
