Rehab Without Walls is a pioneering neuro rehabilitation program that takes individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
Key Responsibilities- Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders
- Regularly reassess treatment plans, attend staffing meetings, and family conferences
- Perform discharge evaluations, make recommendations for home and community safety, and provide referral and education resources
- Communicate patient needs and progress to the treatment team, physician, person receiving services, and family members
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes
- Master's degree from an accredited Speech Language Pathology program
- Valid Speech Language Pathology license in the state(s) of practice
- Current CPR certification in accordance with state regulations
- Minimum one year's work experience as a Speech Language Pathologist
- Demonstrated knowledge of rehabilitation techniques related to complex neurological injury preferred
- Effective communication and professionalism in verbal and written interactions
- Ability to lift 50 pounds and perform physical demands of the job
