Job Title: LPN (Licensed Practical Nurse)

Job Summary: We are seeking a compassionate and skilled LPN to join our team as a Wellness Nurse. In this role, you will provide direct care to residents, promote their physical, personal, and emotional well-being, and work collaboratively with our interdisciplinary team to ensure the highest level of care.

Responsibilities:

  • Provide direct nursing care services to residents, following individualized plans of care.
  • Perform resident assessments, observe changes in condition, and report to supervisors and directors as needed.
  • Document all pertinent observations, interventions, and communication with residents and families.
  • Attend interdisciplinary care meetings, hold problem-solving stand-up communications, and check and record vital signs, weights, and blood sugars as assigned.
  • Assist residents with activities of daily living, provide restorative nursing techniques, and support hydration programs and good dietary intake.
  • Encourage residents to participate in scheduled community activities and assist in planning and implementation of resident activities as needed.
  • Respond to urgent calls, maintain a restraint-free environment, and support fall prevention programs.
  • Communicate with families, report concerns to supervisors and directors, and share information with staff to improve care delivery.
  • Oversee and manage medication delivery systems, including timely order entry, receipt, and tracking, as well as daily MAR documentation and narcotic counts.
  • Attend in-service classes, staff meetings, and orient new associates, leading and coaching certified nursing assistants to improve performance.

Requirements:

  • High school diploma or equivalent (GED)
  • Licensed Practical Nurse/Registered Nurse in the applicable state
  • CPR, First Aid
  • At least 1 year of related work experience

Working Conditions:

  • Works primarily indoors in a climate-controlled setting
  • Possible exposure to outdoor weather conditions
  • Possible exposure to chemicals as identified in the MSDS Manual
  • Continuous exposure to clients and/or family members who may be under stress
