As a Home Health Nurse RN/LVN, you will play a vital role in delivering compassionate and skilled nursing care to our patients, while also working collaboratively with our interdisciplinary team to ensure seamless care coordination.

Responsibilities:

* Assess patients\u2019 physical, psychosocial, and environmental needs to develop personalized care plans
* Implement and document care plans to ensure quality and continuity of care
* Provide skilled nursing care to patients in their homes, utilizing infection control measures to protect both staff and patients
* Supervise and provide clinical direction to Home Health Aides/LVNs to ensure quality and continuity of service
* Monitor assigned cases to ensure compliance with third-party payer requirements
* Demonstrate commitment, professional growth, and competency

Requirements:

* Current State License as a Registered Nurse or Licensed Vocational Nurse
* Current CPR certification
* Ability to document electronically

Job Types: Part-time or Per Visit

Pay: Based on Availability
