Job Title: Pharmacist

Actalent is seeking a skilled Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for ensuring the safe and effective use of medications in our facilities.

Key Responsibilities:
  • Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
  • Recommend therapy changes to physicians and facility staff as needed.
  • Maintain patient/resident databases with current and past medical history, clinical and laboratory data, and concurrent medications.
  • Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
Requirements:
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
