Job Description

As a Nurse Practitioner at Abode Care Partners, you will provide evidence-based medical care to seniors with complex needs, working collaboratively with caregivers and healthcare professionals. This role requires an active NP license, strong communication skills, and a commitment to enhancing the quality of life for patients.

Responsibilities
  • Perform comprehensive patient assessments, develop, and implement individualized care plans.
  • Provide routine and acute care services, including vaccinations, medications, and treatments.
  • Order and interpret laboratory and diagnostic tests as necessary.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Qualifications
  • Active state NP licensure.
  • Current NP National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience, with at least two years of NP experience preferred.
  • Strong interpersonal and communication skills, both verbal and written.
  • Excellent time management and organizational skills.
  • Demonstrated proficiency in electronic medical records (EMRs) and other computer-based data management systems.
