POSITION OVERVIEW:
The Clinical Assessment Nurse (CAN), under the supervision of the Director of Nursing, is tasked with overseeing the continuum of patient care from the point of entry to discharge.

KEY RESPONSIBILITIES:
1. Assessing and monitoring resident care services.
2. Managing the Resident Assessment Instrument (RAI) process.
3. Accurately completing the Minimum Data Set (MDS) and formulating the interdisciplinary Plan of Care.
4. Observing patient/resident conditions to detect changes, obstacles to discharge, and progress towards care objectives.
5. Engaging in daily interdisciplinary team meetings focused on Medicare Prospective Pay System (PPS).
6. Fostering and maintaining constructive relationships with all healthcare team members, functioning as an integral part of the nursing management team.

QUALIFICATIONS:
1. Must possess a current, unencumbered RN license in the state of practice.
2. A minimum of 2 years of direct nursing experience is required.
3. At least 2 years of experience in long-term care is necessary.
4. Preferably, a minimum of 1 year of experience with RAI.
5. Proficiency in ICD-10 coding is essential.
