Pediatric Transplant Coordinator

As a key member of the Health Care Team at Seattle Children's, the Pediatric Transplant Coordinator plays a vital role in the care and support of our patients and families. This role is responsible for coordinating the clinical aspects of solid organ transplant patient care, from evaluation to post-transplant care.
Responsibilities- Assess, plan, implement, and evaluate patient care in a collaborative manner with the patient and their family, significant others, and members of the health care team.
- Coordinate the clinical aspects of solid organ transplant patient care, including pre-transplant evaluation, post-transplant care, and ongoing management of transplant patients.
- Collaborate with the multidisciplinary transplant team to ensure seamless care and communication.
- Develop and maintain effective relationships with patients, families, and healthcare providers to ensure high-quality care and patient satisfaction.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate of an accredited school of nursing; BSN preferred.
- Current Washington State RN License.
- Current Basic Life Support (BLS) for Healthcare Providers certification.
- Previous acute and/or ambulatory solid organ transplant experience preferred.
- Bachelor's degree in Nursing.
- Case management experience.
- Certified Clinical Transplant Coordinator (CCTC) certification.
