Job Summary: We are seeking a skilled Rehabilitation Nurse to join our team at Lourdes Medical Center. As a Rehabilitation Nurse, you will play a vital role in assisting individuals with disabilities and chronic illnesses to attain and maintain maximum function.

Key Responsibilities:

  • Complete referrals and take physician's verbal orders in a timely manner.
  • Perform physical assessments, evaluate patient needs, and consult with physicians to develop plans of care.
  • Assess the need for additional health or ancillary services and refer patients as necessary.
  • Assess family and home environments and make recommendations to enhance patient care and promote safety.
  • Coordinate multidisciplinary care teams and plan patient care.
  • Interpret admission, discharge, and other policies as needed.
  • Provide clear, written, and verbal updates on Home Health Aide and LPN assignments.
  • Provide skilled nursing care and assessments to patients in rehab environments, according to plans of care.
  • Inform patients' physicians and podiatrists about patients under care.
  • Initiate preventive, rehabilitative, comfort, and pain management nursing procedures.
  • Explain diagnoses and treatment plans to patients and families.
  • Perform specialized care in accordance with competencies and demonstrate initiative in learning new skills.

Requirements:

  • Graduate of an accredited nursing program.
  • Current nursing license, in good standing.
  • Minimum of one year current rehabilitation/skilled nursing experience.
  • Current AHA BLS, ACLS if unit consists of Telemetry.
