Job Summary

Cynet Health is seeking a skilled Rehabilitation Nurse to join our team. As a Rehabilitation Nurse, you will play a vital role in assisting individuals with disabilities and chronic illnesses to attain and maintain maximum function.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing care and assessments to patients in rehabilitation environments, including physical assessments, evaluations, and documentation.
  • Coordinate with multidisciplinary care teams to plan and implement patient care, ensuring timely and effective interventions.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with industry developments and best practices in rehabilitation nursing, applying this knowledge to improve patient care and outcomes.
Requirements
  • Graduate of an accredited nursing program.
  • Current nursing license in good standing.
  • Minimum of one year of current rehabilitation/skilled nursing experience.
  • Current AHA BLS, ACLS if unit consists of Telemetry.
