Responsibilities
- Provide primary care services to patients in their homes, working collaboratively with our Medical Assistant partners to deliver exceptional patient care.
- Develop and implement personalized care plans that address the unique needs and goals of each patient.
- Conduct thorough assessments and evaluations to identify potential health risks and develop strategies to mitigate them.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay up-to-date on the latest best practices and research in home-based primary care.

Requirements
- Licensure as a Nurse Practitioner in the state where our office is located.
- Directly related experience in home health, geriatrics, hospice, and primary care.
- Geriatric training or experience preferred.
