Responsibilities

  • Provide primary care services to patients in their homes, working collaboratively with our Medical Assistant partners to deliver exceptional patient care.
  • Develop and implement personalized care plans that address the unique needs and goals of each patient.
  • Conduct thorough assessments and evaluations to identify potential health risks and develop strategies to mitigate them.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and research in home-based primary care.

Requirements

  • Licensure as a Nurse Practitioner in the state where our office is located.
  • Directly related experience in home health, geriatrics, hospice, and primary care.
  • Geriatric training or experience preferred.
