Job Summary: As an Ambulatory Care Specialist, you will serve as a patient advocate, liaison, and advisor/educator to assist patients with navigating the continuum of care.
Key Responsibilities:
- Round on and review assigned patients regularly, evaluating patient progress and communicating the plan of care to patients and families.
- Provide emotional support, counseling, clinical education, and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.
- Serve as a liaison between patients and clinical staff, administration, physicians, managed care companies, and community/external resources to coordinate and maximize resources and identify and resolve barriers to the plan of care.
- Assist patients and families with resolving financial, psycho-social, functional, and administrative issues by advising of options and referring to appropriate resources.
Requirements:
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years experience.
- 2 years of nursing experience.
